1. DESCRIPTION OF AWARD
The award, sponsored by 3M Canada, Inc., is to recognize a Chapter's
achievement in promoting infection prevention and control in the
hospital and the community. The $1,500 award is to cover the costs
of a seminar and/or other educational event.
2. CRITERIA FOR JUDGING
2.1 Originality of the project(s)
2.2 Statement of goals and achievement
2.3 Involvement with the community
2.4 Support of and involvement with CHICA–Canada National
2.5 The effective use of available resources
3. GUIDELINES FOR SUBMISSION
Submissions should include:
3.1 the previous calendar year's goals and achievements for
the chapter as a group;
3.2 as much detail as possible for each of the criteria for
judging. For example, seminars conducted, dates of events, speakers,
relevance to infection control, brochures, programs, educational
material, photographs of projects and displays;
3.3 the geographic region represented, the number of members,
as well as the level of participation of the general membership.
4. PROCEDURE
4.1 The CHICA–Canada President-elect will provide information
regarding the Chapter Achievement Award to the membership by:
- placing an announcement in the Association News Section of
the Fall and/or Winter issue of the Canadian Journal of Infection
Control;
- circulating the guidelines to all Chapter Presidents in November
and a reminder in the new year.
4.2 Four copies of the submission must be sent to the Secretary
/ Membership Director by March 1 to facilitate distribution to
and review by the judges; and a decision prior to the annual conference.

4.3 The winning chapter will be announced at the annual conference;
and, the original of the submission will be displayed at the CHICA–Canada
booth.
4.4 The 3M Canada Representative and Secretary / Membership
Director will make the presentation to the winning chapter and
provide the details on claiming the award.
4.5 The original submissions will be returned to the chapters.
One copy of the winning submission will be retained by CHICA–Canada
national for display at the CHICA–Canada booth each year.
4.6 A copy of the winning submission will be kept in the archives.
4.7 Within six months of winning the award, the chapter must
submit to the Professional Services Advisor, Medical Surgical
Products Division, 3M Canada Inc., London, Ontario, the objectives
and outline, including dates of the education event(s) to which
the funds will be used. The funds must be used within one year
of the award being won.
4.8 If no chapter submissions are received by the deadline,
the award will be made available to CHICA–Canada National for
a specific educational event as described in #6.

Judges who are members of participating chapters will be replaced
at the discretion of the Secretary / Membership Director.
6. JUDGING PROCEDURE
6.1 The CHICA–Canada Secretary/Membership Director will provide
each judge with a copy of each submission, the criteria, guidelines
for submissions and a judging form (see Form #12).
6.2 Each judge will review the submissions, considering each
of the judging criteria and ranking them in order of merit (first,
second, third, etc.).
6.3 The completed judging form will be returned to the Secretary
/ Membership Director by March 31.
6.4 The Secretary/Membership Director will collate the responses
to identify the winning chapter.
6.5 In the event of a tie, a Chapter President randomly selected
from those chapters not participating in the competition will
operate as a fifth judge.
6.6 A compilation of the scores will be returned to the chapter.
6.7 Judging forms will be destroyed after 30 days.
